Apex Performance & Health ("we," "us," "our") is a regulated health-care clinic located at 1020 Brevik Pl, Unit 5, Mississauga, ON. This policy explains how we collect, use, disclose, and safeguard your personal health information (PHI) in accordance with Ontario's Personal Health Information Protection Act (PHIPA).

What information we collect

• Contact form submissions — name, phone, email, service interest, and any free-text message you provide.
• Appointment records — scheduling, clinician assignments, and billing information stored in Jane App (our practice-management platform).
• Clinical records — assessments, treatment notes, and outcomes from your visits, held by your treating clinician.
• Technical data — standard server logs (IP address, user agent) for security and diagnostics. We do not run third-party behavioural-tracking cookies.

How we use it

• Schedule, deliver, and follow up on care.
• Bill your insurance or process direct payments.
• Respond to inquiries submitted through our contact form (we reply within one business hour during clinic hours).
• Meet regulatory and legal obligations (PHIPA, the colleges of physiotherapy/chiropractic/massage therapy, Revenue Canada).

Who sees your information

PHI is only shared with your treating clinician(s), our administrative staff for scheduling and billing, and third parties you explicitly consent to (e.g., insurers you've authorized for direct billing). We never sell or rent your information.

How long we keep it

Clinical records are retained for the retention period required by each regulated profession's college (typically 10+ years after the last visit). Contact form submissions submitted through our website are retained for 90 days then purged.

Your rights

Under PHIPA you may request access to your PHI, request corrections, withdraw consent for specific uses, and lodge a complaint with the Information and Privacy Commissioner of Ontario. To exercise these rights, email us at contact us or call (905) 481-4972.
